Health Information Privacy Notice

What health information we collect, how we use and share it, the rights you have with respect to it, and how to reach us.

Sensible Doc LLC ("SensibleDoc," "we," "us," or "our") respects the privacy of your health information and is committed to protecting it. This Notice explains what health information we collect in the course of providing telemedicine services, how we use and share it, the rights you have with respect to it, and how to reach us with questions or concerns. It applies to the health information we create, receive, and maintain when you use our services.

For purposes of this Notice, "health information" — also referred to in this Notice as "Protected Health Information" or "PHI" — means individually identifiable information about your health, the care we provide to you, and the payment for that care, including your medical history, the information you submit during intake, and the treatment decisions made by our physicians. We handle this information with the protections described below regardless of how it is characterized under any particular statute.

1. What we collect

The Protected Health Information (PHI) and related personal information we collect and store consists of the following:

Medical records.

Patient-reported health history, current medications and allergies, the answers you provide during intake, any lab results or documents you upload, and the clinical information generated during a physician’s evaluation, treatment, and prescribing decisions.

Payment information.

Sensible Doc LLC is a cash-pay practice. We do not submit health insurance claims and do not collect health plan or health insurance numbers. We do not collect or store complete payment card numbers: card details are entered directly with our payment processor, which returns a token, and we retain only that token reference together with charge amounts, dates, and processor transaction identifiers needed to service your membership and support refunds.

Personal identifiers.

Your name, date of birth, home and shipping address, phone number, and email address. We do not collect Social Security numbers.

Device and technical information.

IP address, browser and device information, and similar technical data generated when you use the Platform, including in the security and access logs we keep to protect your records. We do not collect biometric identifiers or vehicle identifiers.

2. How we use and share your health information

We use and share your health information for the following purposes.

To provide your care.

Your intake information is reviewed by a licensed physician who evaluates your request and makes a clinical determination. When a physician issues a prescription, we transmit it, together with the information necessary to fill it, to a licensed pharmacy for dispensing. We may share relevant information with other health care providers involved in your care.

To process payment.

We use your information to process your membership or consultation fee and, where applicable, the charge for a prescribed medication. We share the information necessary to complete these transactions with payment processors and financial institutions.

To operate our practice.

We use your information for the internal activities necessary to run the practice and maintain quality of care, including quality review, provider credentialing, compliance, and administration.

With service providers.

We share your information with third parties that perform services on our behalf, such as our electronic health record and prescribing platform, our telehealth infrastructure, our payment processors, and our hosting providers. We require these providers, by written agreement, to protect your information and to use it only for the services they perform for us.

When required or permitted by law.

We use and share your information where required by applicable law, and where permitted by law for purposes such as public health activities, responding to lawful requests from government authorities, cooperating with health oversight or regulatory bodies, complying with a court order or lawful legal process, and preventing a serious and imminent threat to your health or safety or that of others.

With your direction or authorization.

We share your information with anyone you direct, and for any purpose you authorize in writing. You may revoke a written authorization at any time, and the revocation will apply to future sharing except where we have already acted in reliance on it.

We do not sell your health information, and we do not use it for marketing without your authorization.

Please note that a dispensing pharmacy that fills your prescription maintains your information under its own privacy obligations, which are independent of this Notice.

3. Your rights regarding your health information

You have the following rights with respect to the health information we maintain about you. To exercise any of them, contact us in writing using the information at the end of this Notice.

Access and copies.

You may inspect and request a copy of the health information we maintain about you, including in an electronic format where available. We will respond within the time required by applicable law and may charge a reasonable, cost-based fee for copies as the law permits.

Amendment.

If you believe information we maintain about you is incorrect or incomplete, you may request that we amend it. Your request must be in writing and state the reason for the amendment. If we decline, we will explain why in writing and allow you to submit a statement of disagreement to be kept with your record.

Confidential communications.

You may request that we communicate with you about your health information by a particular means or at a particular location. We will accommodate reasonable requests.

Restrictions.

You may request that we limit how we use or share your health information. We will consider your request, though we are not required to agree to every restriction.

A copy of this Notice.

You may request a paper or electronic copy of this Notice at any time.

Notification of a breach.

You have the right to be notified, as required by applicable law, if a breach affects the security or privacy of your health information.

4. How to file a complaint

If you have a concern about how your health information has been handled, we want to hear from you. You may file a complaint with us in writing using the contact information below, describing your concern in as much detail as possible. We will review every complaint we receive.

You may also file a complaint with the appropriate authority in your state, such as your state attorney general's office or the medical licensing board in the state where you are located.

We will not retaliate against you, and you will not lose any services, for filing a complaint.

5. Our regulatory status

Sensible Doc LLC does not conduct the standard electronic transactions that would make it a "covered entity" as that term is defined at 45 C.F.R. § 160.103, and it is not a covered entity. This Notice is not provided under the HIPAA Privacy Rule, and Sensible Doc LLC does not represent that it is subject to HIPAA.

We nonetheless use the term "Protected Health Information" in this Notice, and extend the protections described here to that information, because the information you share with your care team is sensitive and we believe you are entitled to know how it is handled. Our obligations arise under applicable state medical-privacy and consumer-protection law, our physicians’ professional and ethical duties of confidentiality, and the commitments we make in this Notice. Nothing in this Notice is an admission that Sensible Doc LLC is a HIPAA covered entity or business associate, or that the information described here is Protected Health Information within the meaning of HIPAA.

6. Our duties and commitments

We are committed to protecting the confidentiality and security of your Protected Health Information. Our licensed physicians are bound by the professional and ethical obligations of confidentiality that govern the practice of medicine. We handle your health information in accordance with applicable state and federal law, and we maintain reasonable administrative, technical, and physical safeguards designed to protect it. We will abide by the terms of the Notice currently in effect.

7. Changes to this Notice

We may update this Notice from time to time. If we make a material change, we will post the revised Notice on our website with a new effective date and make it available to you on request.

This Health Information Privacy Notice is effective as of July 1, 2026 and applies to the Protected Health Information maintained by Sensible Doc LLC. Last updated July 24, 2026 to describe the categories of information we collect and our regulatory status.
